Self Referral
Though most of our patients are referred to us by their physician, there are times when a patient decides to make a self referral. As with all patients, our physicians must have certain information to provide you a thorough and accurate medical opinion. This information includes:
- Physician office notes
- Pathology reports
- Radiology reports
- Surgical reports
- Correspondence records
To make a self referral, call 334-528-4411 or 334-528-1070. Choose option 3. The nurse will ask that you bring the aforementioned items to the Cancer Center. Those items are reviewed by the physician, and a consultation appointment is made as warranted.
Please note that some insurance plans require that a referral be made only by a physician. Each patient should review their individual policy.
